Advance Care Planning (Level 2) - For Now and For the Future

Purpose

To enable participants to develop their understanding of  the advance care planning process in Northern Ireland and to prepare them to initiate, facilitate and record Advance Care Planning (ACP) discussions.

Learning Outcomes

• Reflect on personal ACP experiences to enhance future practice. 
• Evaluate legal and ethical principles in ACP decision-making.  
• Apply the four ACP components to clinical case scenarios.
• Facilitate ACP conversations using effective communication strategies.
