Diabetes Management: A Person Centred Approach


Purpose

To provide healthcare professionals an opportunity to increase their knowledge, skills and attitudes in providing person-centred diabetes care, including promoting shared decision making and supporting individuals to manage their own condition effectively.

Learning Outcomes
  • Describe the various types of diabetes and outline evidence-based management plans
  • Discuss the long-term physiological and systemic implications of diabetes on overall health 
  • Examine the psychological, social and emotional impact of a diabetes diagnosis and the lived experience of managing the condition      
  • Develop and adapt individualised care plans in collaboration with the person, incorporating self-management strategies 
  • Share a practice improvement proposal that is directly relevant to your clinical area
